After scheduling, you’ll receive an email with a few brief forms to complete at least 24 hours prior to your consultation. These help us determine whether a provider is a good fit for your needs.
Payment Options
Insurance Accepted
Insurance participation varies by clinician. Please contact us to verify your benefits and clinician availability.
Self-Pay
- $150 per 55-minute session
- $160 Diagnostic Evaluation (55 minutes)
Out-of-Network
We can provide superbills and support submitting them to your insurance provider.
Out-of-Pocket Fees
- Late cancellations and no-shows
- Treatment summaries or documentation
- Paperwork or form completion
- Coordination of care (upon request)
- Court-related services
These services are not covered by insurance and will be charged to the card on file.
Insurance Responsibility
While we verify benefits as a courtesy, clients are responsible for understanding their coverage including deductibles, copays, and session limits.
Deductibles
Clients are responsible for the full session fee until the deductible is met.
